padding: 0px; letter-spacing: 1px; font-size: 16px; max-width: 100%; box-sizing: border-box !important; overflow-wrap: break-word;"> 徐延杰强调，要突出重点、注重实效，推动创建工作深入开展。一要积极开展健康教育。充分发挥党委、政府的号召力，利用多种媒介，在重点区域、重要时间节点，深入开展各种健康公益活动，广泛宣传健康知识，增强防病治病意识和防控能力，引导群众树立健康理念。二要扎实做好基础工作。以“三减三健”活动为切入点，精心指导群众培养健康生活方式；将健身场所纳入城乡建设规划，提高健身场所覆盖率，为慢性病防控打好基础。三要广泛开展健身活动。以开展无烟单位、健康家庭、健康社区、健康学校等创建工作为引领，积极开展群众性健身活动，特别是全县机关事业单位，认真落实工间操制度，定期组织干部职工体检，鼓励职工绿色出行。四要加强居民慢性病管理。完善区域信息平台，实现医疗卫生机构间互联互通、信息共享；全面实行35岁及以上病人首诊测血压制度，强化慢性病筛查，加强慢性病早期发现与管理；稳步推进分级诊疗，全面铺开家庭医生签约服务，逐步成立村级“健康自我管理小组”，强化慢性病患者早期健康干预，推进生命全周期健康管理服务。五要坚持统筹推进。将创建工作与医疗卫生体制改革相结合，与民生实事相结合，与“四城联创”相结合，建立健全政府主导、部门协作、全民参与的综合防控工作机制。</span></p><p><span style="text-align: justify;
